Referring Physicians

Obtaining emergent consultation or transferring patients Dr. Jimenez care at Lutheran General Hospital takes only one call, day or night, to 847-375-3700.

1. Call 847-375-3700
You will request to speak with one of the Physician Assistants during the day.
The answering service will process your call to the appropriate individual in the evening hours.

2. Request a trauma consult with Dr. Matthew Jimenez.

3. In an effort to streamline your transfer, please provide the following information:

  • Direct call back number
  • Patient name
  • Location of patient
  • Diagnosis
  • Name of sending Attending Physician, Surgical Resident, Physician Assistant or Nurse Practitioner to consult with Dr. Jimenez or staff
